Guide to the DATS Application Form

General

Everyone applying for DATS service must submit a completed DATS Application Form.   Each application for DATS registration is carefully reviewed to evaluate eligibility.   DATS service is intended for people who are unable to use regular transit due to a physical or cognitive disability for some or all trips.

Form Design

The application form and the re-certification form were developed recently redesigned as part of the Eligibility Project at DATS.   A task team with representation from clients, administration and healthcare professionals evaluated the old forms and worked with DATS to develop the new forms.  

During the project, we researched other accessible transit properties from across Canada and the US.   Many organizations had very long application forms and many (including Calgary) require a completed comprehensive form and a personal assessment interview.  

We have done our best to develop a comprehensive form that limits the need to schedule personal assessments. Our new form is also designed to be as easy to complete as possible.   The size of the text is larger for easy reading and there are many boxes that can be checked off, instead of needing written answers.

Why So Long?

We are aware that the DATS Application form is quite comprehensive.   To properly assess an application, we do need to understand the applicant's condition and situation in order to determine whether they are eligible for DATS service.  

Everyone who is applying for DATS needs to complete the form, including residents in long term care facilities, visitors to Edmonton, and people wanting to apply for temporary DATS service.   Whether it is for short term or long term use, we still need all the information to evaluate the application.

Completing the Form

All applicants must complete Part A.   Part B must be completed and signed by a qualified health care or social services practitioner familiar with your case, such as a registered nurse, registered psychiatric nurse, occupational therapist, physical therapist, rehabilitation practitioner, social worker or medical doctor.   Charges for completing this form (or for obtaining additional information) are the responsibility of the applicant.  

Sending in your form?

  • Only applications with Parts A and B fully completed and signed will be reviewed.
  • Incomplete application forms will be returned.
  • If your application is approved, your registration card will be sent within 2 to 3 weeks.
  • All information must be kept current.
  • We need all emergency contacts filled out.
  • Call DATS Customer Care Centre at 780-496-4567 option 3 immediately with information changes.
  • PRINT clearly.
